There’s a lot to do once your baby arrives. Preparing a birth certificate is just one of the vital steps in this process. Someone from the Health Information Department at Women’s & Children’s Hospital will contact you for information regarding your baby, information that is necessary to complete this document.
Some of the items you will need include:
- Your baby’s name
- Information for both parents
- Full name
- Date of birth
- Social Security Number
- Highest level of education completed
- Place of birth
Once the information you provide is entered into the computer system, the same representative will review the document with you and have you sign the birth certificate. The birth certificate will then go through several more steps before you receive a certified copy. Inquire with your Health Information Department representative for more information or if you have any questions.
